About this Opportunity

Reporting directly to the Manager Primary Health Care, as the Community Health NurseChronic Disease, you will effectively support the identification of patients with complex care and enhance their access to holistic coordinated care. You will be motivated to achieve positive outcomes for people who live in remote and isolated areas of South Australia.

Specifically, you will:

  • Competently perform clinical nursing duties required in the preparation, execution and monitoring of care plans
  • Coordinate care for patients with Chronic Disease and Complex Care Needs 
  • Support patients in learning more about their conditions and self-management
  • Connect patients to appropriate specialists, allied health and support services.
  • Maintain accurate and legally acceptable patient records, ensuring the security of patient documentation at all times
  • Ensure maintenance and compliance of clinical equipment and maintain clinical stock supplies
  • Provide accurate statistical data and reports on clinic activities as required and prepare written reports on special activities as appropriate.
  • Review new and innovative health developments which may be of benefit to patients and other stakeholders.

About You

You are:

  • An experienced Nurse with an interest in community-based nursing, patient education and health promotion
  • An excellent communicator who is passionate about patient care and achieving positive outcomes for people who live in remote and isolation areas
  • Understanding of cultural sensitivity and its application in healthcare
  • A team player with the ability to build collaborative working relationships with internal and external stakeholders.

To be considered for the role you require:

  • General nursing registration with AHPRA
  • Minimum of five years nursing experience, preferably in a community-based role. 
  • Demonstrated understanding of the needs of Primary Health Care and interest in community-based nursing. 
  • Knowledge of current immunisation schedules, diabetes management, asthma/COPD management, cardiovascular and renal management and other chronic conditions, infection control, CPR and emergency resuscitation techniques, equipment and drugs.
  • Current and valid driver’s licence issued from an Australian State or Territory
