Your Health Navigator is seeking to expand our team of dynamic Registered Nurses who provide Hospital in the Home and Rehabilitation in the Home services to our clients. We are seeking part-time or full-time employees. Availability to work weekends is essential.
Our clinical staff are key to the ongoing success and growth of Your Health Navigator, as we work towards providing our clients with a better healthcare experience and making a positive impact on their lives.
Who are we?
- We are an expanding organisation comprising of 30 staff members
- Our nursing team coordinate and deliver post-acute in-home services for our Hospital Substitute programs - Hospital in the Home and Rehabilitation in the home, on behalf of private health funds and hospitals
- Our nurses provide clinical services such as complex wound care, IV antibiotic therapy, stomal therapy as hospital substitute treatments and provide case coordination and nursing support in conjunction with our allied health team for rehabilitation in the home programs
- We work in collaboration with hospitals and private health funds to tailor programs to meet individual needs
- We identify suitable resources, and refer to appropriate health care providers for ongoing health care needs where required
- We support clients to achieve the best possible health outcomes

Who are you?
- You’re thoughtful and genuinely want to improve the health care experience of our clients
- You're available to work across 7 days per week
- You have experience in community healthcare and/or post-acute nursing
- You’re looking for strong workplace relationships built on trust, respect and integrity
- You enjoy being busy and feeling challenged
- You have excellent communication skills
- You’re adaptable and can move easily with change
- You can work autonomously with excellent time management and organisational skills
- You have sound clinical decision making skills
- You have reasonable knowledge of digital and technological programs such as Office 365, Outlook, Teams or Zoom and an aptitude for learning to use an electronic client record system
- You are a qualified Registered Nurse with current AHPRA registration
- You have your own car and driver’s license (fleet cars are also available and are used on a roster basis)
As a member of our Hospital in the Home and Rehab in the home teams at Your Health Navigator, a typical day will include:
- Visits to clients in their homes and provision of care as requested by referrers, care plan preparation and follow up, and post-acute phone follow up
- Encouraging clients and their families, and safely supporting their success in their home environments
- Ample time for communication with colleagues and for client documentation between visits
