Use your clinical skills and knowledge to assist the Putting Families First Program as a Community Health Nurse!
About the Role
The Putting Families First (PFF) initiative supports the Victorian Government’s vision for children and young people to reach their full potential and to advance Aboriginal self-determination as laid out in the Roadmap for Reform: Strong Families, Safe Children. This is a new model of care to improve the safety, health, functioning and well-being of a whole family as well as individual members of that family, in an integrated and holistic way.
Primary Care Connect (PCC), Rumbalara Aboriginal Co-Operative, OzChild, FamilyCare and The Bridge Youth Service have partnered to deliver the PFF model that is a child centred and family focused and considers the needs of Aboriginal and Torres Strait Islander people in the Greater Shepparton Region.
The Community Health Nurse role focuses on enhancing family health, wellbeing, and inclusion by improving access to services, integrating service delivery, supporting the deepening of client-centred care and care planning and engagement.
The role will work towards supporting and strengthening the ability of individuals and families within the local Aboriginal and Torres Islander community to improve health and wellbeing of;
- Families with a mother who has been in custody (including remand) within the past 12 months.
- Families with at least one young person aged 19 or under who has had a Youth Justice Order issued within the past 12 months or where the young person has had other involvement with the youth justice system including diversion and supervised/intensive bail or remand.
The Community Health Nurse will have an understanding of the social model of health, recognising that health outcomes are determined by a range of social, environmental and economic factors.
Key Selection Criteria
The following criteria must be met and addressed in applications for consideration for this position:
Essential
- An Enrolled or Registered Nurse with a Current AHPRA Registration
- Experience working with Aboriginal and Torres Strait Islander clients.
- Well-developed clinical skills and demonstrated ability to support a coordinated approach by working with a range of stakeholders to improve health outcomes as agreed with client.
- Demonstrated understanding and experience in clinical assessments and care coordination.
- Excellent communication skills including an ability to work within an inter-professional framework and collegiate manner.
- Experience using technology to complete daily work tasks.
- Professional and personal alignment with Primary Care Connect core values.
Desirable
- Experience working with clients who been in custody (including remand) or have been involved with the Youth Justice System.
