Community Liaison Nurse at Western Health
Grade 4A YW8 Community Nurse
Full time/Part time permanent role/s available.
Shifts include AM (0730 to 1600hrs), PM (1300 to 2130hrs), with an expectation to flex into night duty if required (2100hrs to 0730hrs).
Western health at Home Team delivers care in the western region community catchment of Western Health.
Our Goal is to support patients to return to and/or remain at home following a change in health or functional ability.
The position is required to demonstrate expert knowledge and skills while practicing along the service continuum.
Including responsibility for, but not limited to: Telephone Triage, Hospital based liaison, phone support, task based, and assessment within the nursing team and as such requires a broad understanding of the scope of the service.
You will be a part of the at Home - Nursing Service Team assisting patients to receive nursing care either at home, a residential facility setting or referral to another @Home service e.g. Allied Health, or hospital.
To be successful in the role you need to
- Have recent clinic skills and general and / or specialised clinical knowledge
- Have strong communication skills – verbal and written
- Be able to work in a busy environment – be good at prioritizing and work ethic
- Have the ability to problem solve and manage workload effectively
