Sydney Local Health District’s Community Health Centres have a long and proud history of delivering care in the community including prevention, early intervention, assessment, treatment, health maintenance and continuing Care Services designed to improve and support the health and wellbeing of individuals and communities.
About the role
The Acute Care Service model of care provides:
1. Mental Health assessments to consumers within the community experiencing acute Mental Health problems (including a rapid response when indicated) and determining an appropriate pathway to care (e.g., community treatment; assist consumers access and admission to inpatient treatment).
2. Short term Mental Health treatment in the community often as an alternative to hospital.
3. In reach to hospital to promote effective follow up post Mental Health admission.
4. Collaborative care for consumers transferring between hospital and community.
5. The model provides short term Mental Health interventions in the community for consumers experiencing Mental Health distress. Once the symptoms have resolved and the presentation is no longer acute the team collaborate with other stakeholders for the most appropriate ongoing care and treatment.
The Mental Health Nurse will practice in keeping with the standardised Sydney LHD position description for the profession in addition to this position description.

- Recognised nursing tertiary qualifications, current nursing registration with AHPRA.
- Demonstrated competencies to provide contemporary Mental Health practice.
- Current NSW Driver's Licence (P2 licence acceptable).

About working for SLHD
This is a Category A position. To be employed by NSW Health you are required to provide evidence of vaccination to comply with the NSW Health Occupational Assessment, Screening & Vaccination against Specified Infectious Diseases Policy (PD2023_022)
All NSW Health workers are required to have completed a primary course (2 doses) of a Therapeutic Goods Administration (TGA) approved or recognised COVID-19 vaccine (except for the Janssen COVID-19 vaccine which is approved by the TGA as a single dose primary course). New applicants must have completed the vaccination course prior to commencement with NSW Health, or provide an Australian Immunisation Register (AIR) Immunisation History Statement certifying the worker cannot have any approved COVID-19 vaccines available in NSW. A NSW Health agency may require further information about the medical contraindication (including but not limited to an Immunisation Medical Exemption form – IM011 form).
Acceptable proof of vaccination is the Australian Immunisation Register (AIR) Immunisation History Statement or AIR COVID-19 Digital Certificate. Booster doses are highly recommended for all health care workers who have completed the primary course of COVID-19 vaccinations.
