Position Overview:
We are seeking a skilled and compassionate Community Nurse to join our dynamic team. The Community Nurse will play a vital role in providing comprehensive nursing care, health education, and support services to individuals and families within our community. Responsibilities:
Provide skilled nursing care to patients in their homes, including assessments, wound care, medication management, and symptom monitoring.
Develop individualized care plans in collaboration with patients, families, and healthcare team members to address their unique needs and goals.
Conduct health assessments and screenings, provide health education, and promote preventive care practices within the community.
Coordinate and collaborate with interdisciplinary team members, including physicians, therapists, social workers, and community agencies, to ensure continuity of care.
Advocate for patients' rights and access to healthcare services, addressing social determinants of health and barriers to care.
Maintain accurate and timely documentation of patient assessments, care plans, and interventions in accordance with regulatory standards and organizational policies.
Participate in quality improvement initiatives, research projects, and continuing education activities to enhance nursing practice and patient outcomes.
Qualifications:
Registered Nurse (RN) license with Valid APHRA
Bachelor of Science in Nursing (BSN) preferred.
Nursing experience, preferably in community or home health settings.
Strong clinical assessment and critical thinking skills.
Excellent communication, interpersonal, and teamwork abilities.
Knowledge of community resources, healthcare delivery systems, and population health principles.
Ability to work independently, prioritise tasks, and adapt to changing patient needs and environments.
CPR certification and other relevant certifications may be required.
Driver’s preferred
