An exciting opportunity exists for an Occupational Therapist to join the highly supportive and experienced Community Occupational Therapy service at Southcare, located in the beautiful Sutherland Shire.
Where you'll be workingSouthcare is an integrated community health service based on The Sutherland Hospital campus, providing quality services and care to the aged population and people with chronic illnesses residing in the Sutherland Shire. The Occupational Therapy team is small, but experienced and will offer support and development in core community OT skills as required. The OT team also works in conjunction with the wider Southcare multidisciplinary team.
What you'll be doingYou will be predominantly working with the aged population, with the aim of optimising function and maintaining clients in their homes to prevent hospitalisation and support discharge from hospital. Some work with clients aged under 65 years, who have chronic illnesses.The vision for South Eastern Sydney Local Health District (SESLHD) is ‘exceptional care, healthier lives’. Additionally, Category A workers are required to receive a booster dose three months after completing the primary course of COVID-19 vaccinations. New applicants must have completed the vaccination course prior to commencement with NSW Health, or provide an approved medical contraindication certificate (IM011 immunisation medical exemption form) certifying the worker cannot have any approved COVID-19 vaccines available in NSW.Acceptable proof of vaccination is the Australian Immunisation Register (AIR) Immunisation History Statement or AIR COVID-19 Digital Certificate. - Current Class C NSW drivers licence or equivalent
- Relevant and recent post graduate experience working in a community setting with the adult aged and disabled population
- Demonstrated experience in prescription of equipment and home modifications.
- Demonstrated high level written and verbal communication, and interpersonal skills, and the ability to proactively engage with patients/clients to enhance service delivery
- Demonstrated ability to work autonomously with minimal supervision and as part of a multidisciplinary team
- Demonstrated ability in time management skills and in managing a busy caseload
- Demonstrated commitment to quality improvement and evidence based practice and a commitment to ongoing clinical and professional learning
