As a Clinical Care Facilitator servicing clients in the Far North Coast areas, you will coordinate the delivery of high-quality care and services that support our customers to remain living at home. You’ll develop genuine connections with our customers and loved ones and assist them to implement innovative strategies that will optimise their health and wellbeing to support independence. You will manage a caseload of clients and will be responsible and setting individual goals that are realistic and achievable. You will implement the latest best practice and evidence-based approaches.
You will also provide guidance and clinical support to Care Facilitators caring for our Home Care Package clients. This position is home-office based with support from local site office in Ballina. What do we need from you?
You will need to be comfortable working on your own in the community, have excellent verbal communication skills and have an empathetic, patient and caring nature. This role does require you to have:
An Australian driver licence, own a reliable motor vehicle with a minimum Third Party Property Damage car insurance as you need to travel to our clients’ homes or you can opt for a motor vehicle
Registered Nurse qualifications with current AHPRA registration
Proven experience working with budget and service delivery parameters
Demonstrated ability to lead and work as part of a team
