Position Purpose
You will be responsible for providing care to clients in accordance with their care plans developed in collaboration with the client and any members of their health care team. This role primarily covers the Sydney North, South West, Western Sydney and Hills regions.
Must have experience or be passionate about one or more of the following:
- Community Health Nursing Services
- Competence in SPC/IDC management
- Complex wound management
- Continence assessment and scripting
- Continence management
- An interest in bowel management systems
- Wound Management
- Be able to work autonomously.
- Willingness for continuous professional development
- Adaptable and accepting of change
- Can think and act on their feet
Key Selection Criteria
- Bachelor of Science in Nursing (BSN) (Master's degree preferred).
- Current unrestricted Registered Nurse (RN) licence.
- Certification as a Community Health Nurse (CHN) or willingness to obtain.
- Strong interpersonal skills, cultural competence, and the ability to effectively engage diverse populations.
- Excellent communication, organizational, and leadership abilities.
- Passion for community-centered care and a commitment to promoting health equity and social justice.
- Demonstrated SCI , wound and continence assessment experience or a willingness to undertake
- Intermediate skills of computer literacy including working with cloud-based software
- Experience with Workflow Max, Employment Hero, Google Suite preferred not essential.
