Role Overview: As a Community Support Worker at D360, you will be pivotal in addressing various community needs through innovative and supportive programs. Your role will encompass assessing clients' needs, planning and implementing support programs, and liaising with community resources to promote holistic wellbeing.
Key Duties:
- Conduct needs assessments and develop tailored educational and support programs.
- Interview and evaluate clients to understand their difficulties.
- Track and report on client progress.
- Refer clients to additional supportive agencies.
- Evaluate community resources for health, housing, employment, and other services.
- Engage with community groups, welfare agencies, and businesses to raise awareness of available resources.
- Provide support and education for families and individuals with disabilities in various settings.
Requirements:
- Qualifications in community services, social work, or related areas.
- Proven experience in a Community Support role.
- Excellent communication and interpersonal abilities.
- Strong organizational skills and the ability to work autonomously.
- Dedication to creating an inclusive and empowering environment.
