The South West Aboriginal Medical Service (SWAMS) is an Aboriginal Community Controlled Health Service based in Bunbury which provides a variety of health services to Aboriginal people in the South West of Western Australia. SWAMS delivers a wide range of primary health and community programs and has a strong growth strategy through partnership opportunities and future community development.
SWAMS is continuing to increase our service to ensure that we can support our eligible older community members in the Peel region. We are now seeking to bring on an additional Senior Community Support Worker to join a growing team in Mandurah.
The primary purpose of this position is to provide support and mentoring to a team of Community Support Workers providing in-home, direct care to the eligible aged members of the community in their own home. The position conducts initial client intake and supports the Community Support Workers to meet the expected standards in providing care including tasks such as domestic assistance, transport, meal preparation, home maintenance, personal care as well as individual and group social support. The position will ensure that in home services are delivered in accordance with Client Care plans, the Community Care Common Standards and the SWAMS Policies and Procedures.
Specific requirements of this position include but are not limited to;
Essential Criteria
- Minimum Certificate III Individual Support or similar, or willingness to enrol into qualification by end of probation period (6 months)
- Experience in working with Elders and/or aged and community care services
- Demonstrated communication and interpersonal skills, including the proven ability to liaise sensitively and effectively with Elders, that provides high quality client service to a diverse range of people in a professional manner while maintaining confidentiality and discretion.
- An understanding of the health and social issues impacting Aboriginal people
- Demonstrate good time management skills - staff must have the ability to work effectively to meet deadlines and work within timeframes set
- Ability to obtain an NDIS Clearance
- WA Drivers licence
Desirable Criteria
- Aboriginal or Torres Strait Islander descent under section 50(d) Equal Opportunity Act.
- Current knowledge of legislative obligations for Equal Opportunity, Disability Services, Occupational Safety and Health and how these impact on employment and service delivery.
