The Role
- Assessing individual personal and social needs of clients who are eligible to receive Community Service assistance
- Developing appropriate service responses based on assessed need, ranging from the creation and the coordination of packages and services to individual clients
- Assisting with the coordination and day to day running of the centre-based activities including respite groups in conjunction with the Manager
- Assisting the Manager with rosters and supervision of staff involved in the provision of direct client services
- Ensuring professional communication with clients, families and our team
- This is a diverse and challenging role, therefore understanding of the Aged Care Standards and experience providing In-home care would be highly regarded
- Some out-of-hours, on call work may apply from time to time
Essential Criteria
- Be willing to travel around the Fleurieu Peninsula on a regular basis
- Your 2022 Influenza vaccination
- Have had both COVID-19 vaccinations and COVID-19 booster (if due)
- Current P2 or higher Australian driver's licence with access to your own, reliable and insured vehicle
- Experience in program coordination in a community care setting or similar
- Experience leading and supporting a team of committed and professional people - working together to achieve the same goal
- Strong computer literacy
Desirable Criteria
- Background in case management or client support
- You have an empathetic nature and are committed to supporting the needs of older people
