At JC Community Services Australia we have a new and exciting position vacant for a Client Services/Client Support Coordinator reporting directly to the Chief Operations Officer.
The Client Services/Client Support Coordinator is responsible for assisting individuals with disabilities and their families/caregivers in navigating and accessing the services and supports available through the National Disability Insurance Scheme (NDIS). The Support Coordinator will work closely with clients to understand their goals and needs, develop individualized support plans, and coordinate the implementation of services.

The Role and Responsibilities include:
• Provide guidance and support to clients in understanding the NDIS processes, eligibility criteria, and available funding options.
• Conduct intake assessments to gather information about clients' goals, needs, and preferences.
• Collaborate with clients to develop individualized support plans based on their goals, preferences, and available NDIS funding.
• Assist clients in identifying suitable service providers and coordinating the implementation of services and supports.
• Advocate for clients in ensuring their access to appropriate and quality services that align with their individual needs and goals.
• Establish strong working relationships with service providers, community organizations, and other stakeholders to facilitate service coordination and collaboration.
• Monitor and review the progress of clients' support plans and make necessary adjustments as needed.
• Provide ongoing support to clients and their families/caregivers, answering questions, providing information, and assisting with any issues or concerns that may arise.
• Maintain accurate and confidential client records, including support plans, progress notes, and related documentation.
• Stay updated on the latest developments, changes, and best practices within the NDIS and disability support sector.
• Operate with a strong understanding of all relevant external legislation & internal policies and procedures that relate to this position JC Community Services Australia.
• Act in a professional manner at all times when dealing with internal & external clients.
• Treat all clients with respect & equality, whilst being responsive to their needs in accordance with program guidelines.
• Comply with relevant JC Community WHS policies, procedures, work instructions and requests.
To be successful in this role you will have the following qualifications and experience:
Essential:
• Demonstrated knowledge and understanding of the NDIS legislation, policies, and processes.
• Experience working with individuals with disabilities, including knowledge of disability support services and community resources.
• Strong communication and interpersonal skills to effectively engage with clients, families/caregivers, and stakeholders.
• Ability to assess client needs, develop support plans, and coordinate services effectively.
• Excellent organizational and time management skills to manage multiple clients and tasks simultaneously.
• Ability to work independently and as part of a team, collaborating with other professionals and service providers.
• Compassionate and client-centred approach to supporting individuals with disabilities.
• Proficiency in computer skills and experience using relevant software and databases for client management and documentation.
Desirable:
A bachelor’s degree in Social Work, Psychology, Disability Studies or a related field will be highly regarded but not essential.
