We walk alongside community members in the spirit of collaboration to provide a supportive and accessible service that listens, and respects cultural and social differences. Our coordinators will support and empower individuals to identify aspirations aimed at maximising their independence, connection to community, and social opportunities.
The Role
The Community Connections Program (CCP) supports people who are 18 to 64 years who not eligible for mainstream programs such as NDIS and My Aged Care, and whose wellbeing is at risk because they are disconnected from necessary supports face social isolation.
Individual and groupwork sessions can be held to assist within the following areas:
- Connecting with like-minded individuals of similar ages, culture and interests through demonstrated ability to build rapport, stakeholder and Community Connections
- Social events and group opportunities.
- Connecting to social opportunities in the local area.
- Connecting with other community services.
- Increasing capacity with areas of daily living.
- Strengthening links with wellbeing and lifestyle providers.
- Sharing information regarding education participation, employment opportunities, and suitable housing options.
This position is funded on a fixed-term opportunity until 30 June 2024.
What you need to bring
Essential
- Minimum Certificate 4 qualification in a relevant field and demonstrated equivalent experience
- Demonstrated ability to build rapport, stakeholder and Community Connections
- Proficient in computer use and ability to learn and use databases
- Strong written and verbal communication skills
- Valid Department of Human Services (DHS) Working with Children Check
- Valid Australian Drivers Licence
Desirable
- Tertiary level qualification in related field
- Experience working with Aboriginal and Torres Strait Islander communities
- Experience working with CALD communities
- Experience working with unpaid carers
- Experience working within the community services sector
Special Conditions
- Flexibility is essential in this role - some work outside of standard hours may be required.
- Willingness to take part in Centacare’s fleet car program or grey fleet your personal vehicle.
