OnTrack Tasmania is an expanding NDIS provider whose purpose is to enrich the lives of people with disabilities. At OnTrack, we have a strong focus on social justice and support people living with mental health or social issues to live as independently as possible in the community. With a focus on Human Rights, we make further referrals to suitable agencies and advocates on their behalf as requested in support of needs being met. Whether it is getting into housing, joining community groups, or gaining access to employment, we are with them every step of the way.

Key responsibilities to be eligible for this role include, but are not limited to:
- Always engage in best practice
- Maintain a workflow of excellence
- Maintain a state of audit readiness
- Understand and provide appropriate support to Participants with low and high needs who demonstrate a range of challenging behaviours
- Assisting Participants to identify preferred supports relating to daily living, community support, and empowerment with attention to individual NDIS Plans
- Work under general guidance from the person seeking support, support network and or their family within clearly defined guidelines
- Perform administrative duties and maintain accurate case records
- Participate in case reviews to determine the needs and well-being of the Participant
- Attend Team Meetings and other meetings as required
- Accurately record daily activities
- Collect and maintain data to ensure accurate and up-to-date information, supporting excellent Participant support both within and external to the Company
To be successful in this role, the following essential criteria are required:
- Relevant Certificate IV, Diploma or Tertiary qualification
- Experience in the Human Services sector in Tasmania and knowledge of a broad range of disability types, including Mental Health Illness.
- High-level organisational and demonstrated computer skills.
- Excellent verbal communication and interpersonal skills, including the ability to negotiate effectively.
- Excellent writing and presentation skills to produce high-quality reports in a timely manner.
- Ability to work with OnTrack Tasmania’s Mission, Values, and Commitment.
- Understanding of and commitment to the principles of person-centred planning.
- Understanding of and the ability to apply NDIS legislation, NDIS practise standards and requirements of the NDIS Quality and Safeguards Commission.
- Current unencumbered Tasmanian driver's licence.
- Meet the NDIS Worker Screen requirements, Working with Vulnerable People clearance, and a current National Criminal Record Check.
- Completion of NDIS worker orientation and Covid-19 Infection Control Module.
- Current Assist clients with Medication Certificate.
- Current First Aid Certificate.
- Covid-19 Vaccine record.
