Cps Case Manager

As a Case Manager for Coastal Supports at Open Minds Sunshine Coast, you'll play a pivotal role in delivering mental health psychosocial support services across the Sunshine Coast region. Working as part of our dedicated team, you'll have the opportunity to develop holistic plans that promote personal recovery, evidence-based approaches, and community connectedness for individuals with mental illnesses. You'll work proactively with clients facing mental health challenges, helping them enhance their social and emotional skills, build family connections, manage daily living needs, and develop vital life skills. Your role will also involve assisting clients in accessing government-funded services, ensuring comprehensive support for their journey towards recovery and independence.

This is a full-time maximum term contract until 29th June 2024 with the possibility for extension.

Key Responsibilities:

• Collaborate with a team to develop and implement holistic support plans for individuals with mental illnesses.
• Provide in-reach and outreach support, promoting individual choice and independence.
• Conduct client needs assessments, develop Personal Support Plans, and coordinate/facilitate group services.
• Maintain accurate client information and documentation, participate in data collection, and contribute to team meetings.
• Uphold Open Minds' values, engage in professional development, and demonstrate effective interpersonal skills.
   

About you:

• Tertiary degree or Cert IV in a mental health-related field, along with significant experience in health, community, or social service sectors.
• Previous experience in mental health services and/or disability support.
• Strong passion for team-driven positive change for individuals with severe mental illness.
• Capability to work independently and collaboratively, with above-average organisational skills for effective CPS program delivery.
• Strong written and verbal communication skills, conflict resolution abilities, and excellent interpersonal skills.
• Proficient in developing individual support plans.
• Proven ability to engage, liaise, and network with stakeholders, fostering community relationships and collaboration with service providers.
• Readiness to assist complex clients facing diverse challenges.
• Understanding of relevant legislative requirements and regulations.

• Current Police Check
