Pro-active Health is seeking a skilled Credentialled Diabetes Educator. The role presents an exciting opportunity to make a significant impact on the lives of our veterans through the convenience and accessibility of telehealth services.
As a Credentialled Diabetes Educator, you will play a crucial role in our mission to provide comprehensive diabetes education and support to DVA recipients across Australia. Your primary responsibility will be conducting telehealth consultations, educating patients on diabetes self-management, and empowering them to make informed decisions about their health.
Starting at 8-16 hours per week, it is envisioned that the role’s hours will steadily increase.
Responsibilities:
- Conduct telehealth consultations for assessment, completion of a patient care plan and implementation of The National Diabetes Education Program.
- Provide diabetes education and support, including information on nutrition, medication management, blood glucose monitoring and lifestyle modifications.
- Provide tailored recommendations based on patient’s goals, preferences and literacy.
- Refer clients to Pro-active extensive network of GPs, Psychologists, Psychiatrists, Dietitians, Exercise Physiologist, Podiatrists and to Case Conferencing, as needed.
- Maintain accurate and detailed patient records for clients’ progress notes, patient care plan, end of cycle report and DVA claim item numbers for every consultation, every time.
- Provide service aligned with DVA guidelines, relevant professional standards and codes of conduct.
Requirements for the role:
- Current ADEA registration as a Credentialled Diabetes Educator
- Current certification with practising professional body (E.g. AHPRA, ESSA, DAA)
- Minimum 3 years’ experience working as a CDE
- Excellent interpersonal and communication skills.
- Proficient in using digital tools and software for remote work and telehealth consultations.
