We are seeking a skilled telehealth Dietitian | Diabetes Educator to join our team. The role presents an exciting opportunity to make a significant impact on the lives of our veterans through the convenience and accessibility of telehealth services.
Working remotely, you will be required to work 8-16 hours per week as part of Pro-active Health’s Case Conference panel. The aim of Case Conferences is to improve client health outcomes by fostering collaboration and communication among healthcare professionals. These conferences bring together multidisciplinary teams of doctors, nurses, therapists, and specialists to discuss complex cases and devise comprehensive treatment plans.
Responsibilities:
- Participate in Case Conferences with a team of healthcare professionals to discuss and plan client care.
- Provide education on Diabetes and expert advice regarding good nutrition including recommendations for dietary modifications.
- Assess nutritional needs, diet restrictions, and current health plans to develop and implement dietary-care plans.
- Monitor the effectiveness and impact of dietary plans and make changes as needed.
- Maintain accurate and timely client records, including case notes and treatment plans.
Requirements for the role:
- Current Dietitians of Australia (DA) Registration.
- Completed Accredited Practising Dietitian (APD) program.
- Tertiary qualification in Nutrition and Dietetics.
- Current ADEA registration as a Credentialled Diabetes Educator
- Minimum of 3 years of clinical experience, preferably in a multidisciplinary setting.
- Excellent interpersonal and communication skills.
- Ability to work effectively in a team and independently.
- Proficient in using digital tools and software for remote work and telehealth consultations.
