DIABETES NURSE EDUCATOR

Casual

Available from 8th April 2024 to 2nd June 2024

An exciting and rewarding opportunity is available for a suitably qualified Diabetes Nurse Educator to join the Primary Care team at Portland District Health.

The primary role is to be responsible for delivering safe and effective clinical care that requires a higher level of clinical decision making that is relevant to your speciality area of practice. You support the development of clinical and leadership skills in staff placed under your responsibility and less experienced nurses. 

About you:

  • Registered Nurse with Australian Health Practitioners Regulation Agency and holds a current registration with a minimum of three years’ experience in relevant clinical area.
  • Post Graduate clinical qualification(s) in Diabetes Education and currently credentialed to practice as such.
  • Extensive clinical knowledge, skills, and leadership experience in nursing.
  • Must be a credentialed Diabetes Nurse Educator
  • Demonstrated sound advanced clinical assessment and analysis skills and knowledge, and ability to implement indicated treatments in speciality area of practice.
  • Demonstrated excellent communication skills, written and verbal.
  • Understanding of relevant accreditation and funding systems and demonstrated ability to manage continuous improvement portfolio.
  • Demonstrated computer literacy, including ability to access and utilise organisational IT systems and emerging technologies which are relevant to the completion of clinical care.
  • Demonstrated understanding of the needs, issues, and sensitivities of people from diverse backgrounds
  • Understanding of opportunities and challenges associated with providing rural health care services.

Portland District Health (PDH) provides an integrated health service for the community. The hospital was established in 1856 on the current site and now provides a comprehensive range of services that include:

  • Hospital based Medical and Surgical services, Medical Imaging (Radiology), Pharmacy and Pathology
  • A 24/7 Urgent Care Centre for emergency treatment
  • Residential aged care for 30 residents at Harbourside Lodge
  • An extensive range of primary and community services for the local community

PDH delivers services from a single campus, employs around 470 staff with an FTE of approximately 290, and has an active volunteer workforce. The People and Culture Unit sits within the Corporate Services Directorate, with other Units including Finance, Building Services, Food Services, Environmental Services, Executive Support, Administration and Health Information.
