About the role
South West Healthcare (SWH) is seeking and experienced Diabetes Nurse Educator to join our Community Health team. The Community Health Team are part of the Primary and Community Services Division and provides a key service to support clients across the organisation from the impatient setting through to clients living in the community. The service extends across all South West Healthcare campuses in addition to contracted services outside the organisation.
As Diabetes Educator, you will provide clinical expertise, including being a resource person in the delivery of nursing care with the areas of Diabetes management and liaise with other professional and community groups in planning throughout the continuum of care.

Salary: $107,962 pro rata, plus superannuation
What you bring
The successful applicant will:
Have a minimum of five years post graduate nursing experience in a relevant speciality.
Have a graduate certificate in Diabetes Education or equivalent.
Be credentialed by the Australian Diabetes Educator Association or working towards
Demonstrate an ability to work both independently and as part of a team, effective communication, organisational and computer skills are also essential.
