About the role:
Uniting AgeWell strives to create and deliver innovative, high quality and accessible services to people through their ageing journey. In doing this, we address the physical, cognitive, emotional, social and spiritual wellbeing of those to whom we provide care and support.
- Drive the implementation of the Uniting AgeWell Dementia Framework and Action Plan
- Run Steering Committees and working groups relevant to the Framework
- Support relevant Uniting AgeWell teams to develop and implement the policy, practice standards, education and training frameworks, outcome tracking, awareness raising, care planning, service programs and referral pathways, activities related to the Framework
- Support operational team change management and implementation of the framework and related projects at the service delivery level
- Ensure the dementia framework aligns with the other strategic Frameworks with emphasis on palliative care and mental health
About you:
You are passionate about what you do and want to make a difference in the lives of our residents/clients, enhancing their self-esteem and empowering them to make choices.
This is the place for you if you:
- Have or be working towards a qualification in Dementia
- Project Management skills and complex project delivery
- Demonstrated change management experience
- Experience in the aged care, dementia, community, or health sector
- Ability to meaningfully engage Uniting AgeWell’s customers and staff, ensuring their voices are heard in the planning, design and implementation and outcomes of the project
- Experience engaging internal and external stakeholders
- Experience developing and writing high quality documents and reports
- Thrive in a team and also appreciate the one to one interactions
- Are committed to being part of a workplace culture based on the BEST way of life (Believe, Excel, Support and Trust)

- A current Working With Children card
- Current and ongoing flu and COVID-19 Vaccinations
- Australian working rights
- Ability to pass a national police clearance
