We are presently working with a well-respected, multi-cultural community service organisation who service their local communities with the provision of quality, responsive and compassionate care.
We currently have an opening for a Dementia Project Coordinator to join their team and be responsible for community engagement in the City of Casey.
Key Responsibilities:
- Improve the wellbeing of both clients living with dementia and their carers, whilst still living at home
- Provide dementia specific respite support services to both clients and carers
- Manage client and carer's needs assessment, reassessment and regular care plan reviews
- Responsible for onboarding new referrals through community engagement and M.A.C
- Facilitate and evaluate group sessions around Dementia
Prerequisites required for the role:
- Strong knowledge of ACAS, RAS and Case Management in aged care community setting
- Ability to liaise with Stakeholders and Community Services
- Project management experience in aged care setting desirable
- High level of motivation, proactiveness and team orientated approach
- Excellent time management, communication and negotiation skills
- Ability to work in a KPI focused environment
- Demonstrated understanding of client's living with dementia and carer needs
- Ability to communicate effectively with people from diverse backgrounds
- Experience with my aged care portal (M.A.C)
- Willing to travel for work when required (car & VIC license is a must)
