ABOUT THE ROLE
As an Intake and Engagement Consultant, you will primarily work with new and existing clients who are living with a disability to ensure you are capturing all relevant details of that client's needs so we at HenderCare can assess their requirements and appropriately coordinate services for them to live happy and independent lives.
On a day-to-day basis, you can expect to:
- Onboarding new participants of the NDIS
- Conduct Intake and assessment of new clients and develop care plans
- Assisting consumers and participants to navigate the NDIS
- Providing effective, responsive, goal focused and outcome-based service provision
- Care Planning – developing, reviewing and monitoring care plans
- Assisting in marketing HenderCare services to participants
- Providing advice and advocacy that supports the promotion of individual rights and choices for consumers and participants
- Reviewing service agreements and support plans
- Liaise with networks and referring agencies

You will also bring:
- Relevant experience in case management of services to people with a disability
- Bachelor or Diploma in Nursing
- Exceptional knowledge of disability services within the NDIS space
- Ability to take initiative and work autonomously
- Able to prioritise work effectively under pressure
- Experience in working with participants with complex care needs
- Valid Working With Children Check
- Valid NDIS Screening Check
- Valid National Police Check
- Valid Drivers License
