We are looking for experienced, reliable, committed, compassionate and empathetic Aged and Disability Workers who want to make a difference by supporting our participants in achieving their goals and living their best life.
More specifically, the role may involve, in home support, social support, transportation to medical appointments, recreational activities and supporting participants with complex needs.
It could also involve personal care, light domestic duties and assistance with meal preparation.
BenefitsTask & Responsibilities- Assist clients in engaging in age-appropriate activities and tasks, including helping them find employment, offering in-home support, and facilitating community access.
- Provide support for clients to meet their personal care requirements as outlined in their service plans.
- Enable clients to access community activities and outings.
- Deliver personal care assistance to each client according to their individualized service plan.
- Encourage community inclusion and participation, taking into consideration each individual's unique needs, capabilities, and preferences.
- Execute assigned tasks and activities as directed, while also displaying initiative as needed to fulfill clients' personal, residential, and community-related needs, team objectives, and program goals.
- Abide by the organization's communication, reporting, and record-keeping procedures, ensuring accurate client records are maintained.
- Foster collaborative and supportive interactions with clients, their family members, and other stakeholders.
- Adhere to the organization's policies related to medication administration and workplace health and safety.
- Actively engage in the development and monitoring of individual program plans, taking responsibility for achieving the planned outcomes.
- Certificate III in Individual Support or equivalent
- Previous experience working with individuals with disabilities
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Current First Aid and CPR certification
