Our Journey
The Monash Health Community Access team is excited to announce the following position that is available for an Enrolled Nurse in Community Access. This position is a fixed term full time parental leave role and presents an excellent opportunity for a like minded nursing professional to join a dynamic team which strives for excellence in Service delivery to our clients. This is a fixed term full time role, working Monday- Friday 0800-1630pm role excluding public holidays, located at Doveton. The role concludes on 13/7/2025.
Monash Health Community proactively supports our community to improve and maintain health, independence and well-being by coordinating and/ or providing integrated, multidisciplinary care across multiple settings within the local community. Our aim is to empower and prepare clients to self-manage their health and health care. Working with clients across the aged spectrum, Monash Health Community provides a continuum of care, supporting clients through all stages of non-acute care from prevention through to rehabilitation.
We provide services that support clients in their homes, community based facilities and dedicated Monash Health Community hubs. Community Access is a dynamic and growing program that is a central referral point for people in our community to be referred for Community Health, Post Hospital Services and Aged Care and Rehabilitation. We provide a telephone based service, and this is a non-patient facing role.
About the Role
As an Enrolled Nurse you will be utilising your clinical skills to ensure our clients are referred, triaged, registered and booked into an appointment with the most appropriate Service.

About You:
You are experienced, flexible and dynamic Enrolled Nurse that is looking for a change in career.
You are familiar with health management data systems, or are a quick learner with a “can-do attitude”
We are a high performing team and we ensure that our client’s journey through Community Health is seamless and timely, it is therefore vital that we have a like-minded Enrolled Nurse to continue this journey with us!

What You Need:
Relevant Qualifications as an Enrolled Nurse, essential
Current Victorian Driver’s Licence
Demonstrated experience working in a rehabilitation, aged care or community setting, desirable
Substantial post graduate experience, essential
Current and unrestricted AHPRA Nursing registration
COVID 19 Vaccination evidence
Australian working rights
