The role
We are looking for a passionate Case Manager to guide the Children, Young People and families we support. We work collaboratively to deliver services through a strength-based and person-centred approach.
As an experienced Case Manager, you will:
- Manage a caseload of children and young people/adults and their families and carers
- Complete comprehensive assessments to ensure the development, implementation and review of individual case plans
- Drive intervention strategies e.g., risk assessments, referrals and program applications
- Apply an understanding of child protection related court procedures and practices, including preparing court testimonies and other legal documentation
- Facilitate access to services and provide feedback on their effectiveness
- Prepare reports and correspondence relating to the implementation and progress of case plans

About you
- Demonstrated experience in case planning for Children and Young People or similar
- Knowledge of the Children and Young Persons (Care and Protection) Act 1998
- Experience in meeting the holistic complex needs of children who have experienced significant trauma and/or a disability, and experience challenging behaviours
- A passion and drive to help support children to create positive outcomes in their lives
- Strong analytical and assessment skills
- Strong written communication and record-keeping skills
- Ability to work flexible hours
- Updated COVID19 vaccinations (at least 2 doses)
- Tertiary qualifications in a relevant field are desirable but not essential
