This is an opportunity to work with one of Australia's leading providers of Out of Home Care to Children & Young People and Disability services in the Greater Sydney region. This NFP is known for their trauma informed, person-centered approach, and their desire to make positive and impactful change for all young people under their care.
The Role
Our client is looking for an experienced Case Manager to manage a portfolio of OOHC clients across Sydney. Working out of their Baulkham Hills office, you'll have experience managing an OOHC clients and be happy to travel out to clients. In return you'll be offered flexible start and finish times, autonomy over your schedule and the ability to work from home 1-2 days a week. You'll be working as part of a passionate, supportive and flexible team who values collaboration and career progression.
Main responsibilities include:
- Manage a caseload of OOHC children and young people/adults and their families and carers
- Complete comprehensive assessments to ensure the development, implementation and review of individual case plans
- Apply an understanding of child protection related court procedures and practices
- Facilitate access to services and provide feedback on their effectiveness
- Prepare reports and correspondence relating to case plans
- Minimum 2 years Case Management experience for Children and Young People in OOHC
- Experience in providing case management to children who have experienced significant trauma
- A desire and enthusiasm to support children in OOHC
- Ability to manage a portfolio of clients including client visits
- COVID19 vaccination x2
- Valid WWCC & NSW Drivers License
- Qualifications in relevant field (desired)
- Salary based on SCHADS Level 5.1
- Salary packaging $15,900 general and $2,650 entertainment p.a.
