What You Will Be Doing
The role of Community Care Geriatrician is to work within the Community Care team, primarily the Residential Inreach program, supporting the almost 50 Residential Aged Care Homes (RACH) in our catchment area. The role will involve undertaking Comprehensive Medical Assessments that support patients to remain optimally cared for in the RACH setting. It will also involve providing advice and support for Aged Care clients cared for by other Departments serviced by Community Care such as Complex/Community Care clients and Hospital in the Home (HITH) patients on an as needed basis.
What You Need
We anticipate you will have:
• Medical Practitioner Registered in Australia.
• Fellow of Royal Australasian College of Physicians as a Geriatrician
• Current and valid Drivers License.
