An exciting opportunity exists for Health Worker/Advanced Health Worker to provide high quality care and support to Aboriginal and Torres Strait Islander patients and their families, as well as cultural education and training for Sunshine Coast Hospital and Health Service staff. The Aboriginal and Torres Strait Islander Health Directorate provides a multidisciplinary team approach to complex care including inpatient support, case management, care coordination through the health service and a supported transition back into primary care.
Sunshine Coast Hospital and Health Service (SCHHS) is in southeast Queensland and extends through the coastal and hinterland areas from Caloundra in the south and Gympie in the north.

Key Responsibilities:
- Knowledge and understanding of Aboriginal Torres Strait Islander culture and ability to liaise and communicate with Aboriginal and Torres Strait Islander individuals' families and communities.
- Work collaboratively under supervision within the multi-disciplinary team to provide quality care, including health awareness, education and health intervention strategies for individuals and the community.
- Deliver a range of evidence-based interventions in a culturally safe, clinically competent, professional and effective manner including the provision of co-case management, care coordination, intake assessments, cultural aspects of assessment and treatment plans, direct therapeutic interventions, referrals and cultural support.
Salary between $58 461 - $62 828 p.a., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HW2) (Fixed Term Temporary Full time position for up to 12 months), or Remuneration value up to $76 670 p.a., comprising salary between $63 921 - $67 198 p.a., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HW3) or Remuneration value up to $86 646 p.a., comprising salary between $80 308 - $85 223 p.a., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HW4).
