Together let's redefine retirement living for senior Australians.The OpportunityIn this role, you will be reporting directly to the Care Manager. Our Case Managers play a critical role in our organisation with the goal to create a happy, welcoming, caring, and safe environment for residents. Servicing mainly Aveo Cleveland Gardens retirement village, you'll proactively deliver exceptional person-centred Home Care Case management for consumers within the community. This is a rewarding opportunity that will enable you to actively achieve quality of life for our consumers while working within a supportive, experienced team.You willProvide information on funding options and services to community groupsMeet with prospective customers and their families to understand and assess their care needsDevelop comprehensive and tailored care plansOversee Case management of resident's Home Care packages including ensuring supports are in place as requiredAssess, review, and evaluate care and services provided to consumers Prepare and maintain records, documents and reports as required Ensure all residents receive general and support services in line with resident contracts and care plans and be accountable for all medication managementMaintain effective inter-personal relationships with residents and wider stakeholders Liaise effectively with external service providers who provide care within the communityWhat you'll bringCurrent AHPRA Division 1 registration (required)Knowledge of Home Care packages or similar commonwealth funding packagesPrior experience in case management (desired)Experience in Community Care or Retirement Living (desired)Current CPR certificate within the last 12 months or willingness to obtainProficient in the use of technology and digital systemsTrustworthy, reliable & enjoy working in a team environment Excellent organisational skills, attention to detail and ability to prioritiseWell-developed interpersonal communication and leadership skillsWhat we offerUp to 12 weeks paid parental leaveAbility to purchase additional annual leaveShort Term Incentive (Annual Bonus Scheme)Aveo Benefits - access to exclusive savings on hundreds of retailers, products & servicesReferral Bonus up to $750 on each successful referralWho we areAs a leader in retirement living, with 30 years' experience, Aveo provides the stability and confidence to try new things and keep innovating.
