We are offering an exciting opportunity for a Home Care Coordinator to join our team.
About the role:
The Home Care Coordinator works in partnership with clients receiving a Home Care package; to co-design a plan of care, based on the client's goals, preferences and assessed needs within an individualised budget.
The Home Care Coordinator is responsible for superior customer service and the delivery of client focused care and services that effectively meets their individual needs, while promoting functional independence, wellness and lifestyles choices within a Consumer Directed Care (CDC) environment. The Home Care Coordinator will liaise with internal and external stakeholders to ensure positive client outcomes within resource/budget allocations.
This role is based at our Lavington office.

Essential Requirements:
- Tertiary Qualification or equivalent experience in Aged care, Community. Services or a health related discipline.
- Experience in the provision of Coordination, Case Management and Care Planning.
- Understanding of current Aged Care Legislation, Aged Care Principles and Home Care Standards.
- Understanding of Consumer Directed Care (CDC).
- Computer literate (Word, Excel, email, internet, electronic client records).
- Current Driving Licence.
- Criminal History Screening (Undertaken by UPA).
- Senior First Aid and CPR Certificate.
- COVID-19 vaccination and current Influenza vaccination.
