About the role
Due to growth, an exciting opportunity has become available in our Home Care Team, managing Home Care staff and clients who are receiving Home Care Package services. We are looking for a permanent full-time Home Care Coordinator to be based at our Rockingham Hub.
The successful candidate will be responsible for the delivery of services to Home Care clients, including government funded HCP and occasional Private/CHSP clients. Your role will include covering Home Care Coordinators when they’re on leave, developing support plans in partnership with your clients, coordinating all client care, liaising with Mercy Care’s clinical and allied health teams, and managing your clients’ budgets. You will work closely with clients, their family, and their support networks to achieve positive outcomes.
You will also lead a team of Support Workers, Senior Support Workers, and Home Care Assistants to deliver quality Home Care services. This will include providing guidance, mentoring, and training for staff, providing learning opportunities and development, and supporting staff to deliver quality care that promotes client choice and independence.

Essential:
Experience guiding staff and managing Home Care packages and/or CHSP or significant frontline experience.
Demonstrated knowledge of service provision, allocation, and compliance with contractual obligations to ensure quality service.
Knowledge of the Aged Care Quality standards
Well-developed communication and interpersonal skills
Demonstrated time management and organisational skills to effectively plan and prioritise work tasks
Demonstrated empathic and flexible approaches to manage the sensitivities of conflict resolution of consumer / employee issues
Proficient in the use of the Microsoft suite of programs including Outlook, Word and Excel with experience using Information Management Systems
Current C Class Driver’s Licence
National Police Clearance (obtained within the last six months)
Desirable:
A Certificate IV in Community Services / Aged Care / Disability, or a Health / Human Services equivalent tertiary qualification is preferred, or significant relevant industry experience.
COVID and Influenza vaccinations are preferred but not required.
Current Basic First Aid Certificate (or ability to obtain)
