We are currently seeking an experienced Home Care Coordinator for a full -time ongoing role based in Ballina, NSW. As a vital member of our team, you will oversee and facilitate the provision of high-quality care and services to our clients and participants.
Monday to Friday 08:30am - 16.30pm
Key Responsibilities:
Perform client care assessments and oversee ongoing care planning
Manage client compliance and maintain a case load of clients
Coordinate the delivery of personalised care tailored to clients' needs and care plans while adhering to allocated funding and budget requirements
Engage in general administrative tasks associated with the role
Desired Skills:
Relevant qualifications Cert IV or Diploma Nursing highly regarded
Prior experience in an aged care/NDIS/mental health care coordination role is advantageous
Familiarity with My Aged Care, Home Care Package, and NDIS systems
Experience in evaluating and planning care needs within the community
Proficiency in using computer systems, including Microsoft Office 365, Sharepoint, Outlook, and rostering software
Proven ability to manage incoming calls in a friendly and professional manner
Demonstrated capacity for planning, organisation, problem-solving, and solution-finding
Positive attitude with a strong aptitude for teamwork
Outstanding networking and consultation skills to establish relationships with external organisations for referrals
Exceptional customer service skills, both written and verbal, while maintaining confidentiality and privacy in accordance with company policies
