Hume City Council's Home Care Package Program is growing, and we have an exciting opportunity for an experienced Care Advisor to join our innovative and supportive team.

What you'll be doing:

  • Hume City Council's Care Advisor role provides culturally appropriate case management and services to Home Care Package clients
  • The Care Advisor is responsible for collaborating with clients to identify, assess, plan, implement, monitor, and review client services and needs, with the aim of supporting individuals to remain living in the community setting
  • Provide appropriate advice and support to clients, enabling them to navigate the service system and empower them to make choices and decisions that best suit their individual needs
  • Actively participate in the implementation of the Continuous Quality Improvement Program as it applies to Aged & Disability Services
  • Collaborate with clients, fostering an understanding of their needs and aspirations in order to explore options and develop actions that will best meet their needs
  • Assist the client to identify and review goals and plan services in line with the client's available budget
  • Oversee the individual client budgets and expenses and provide the client with a monthly statement of expenditure
  • Actively participate in the development, implementation, evaluation and review of Aged & Disability Services policies and procedures.

The skills and experience you'll bring to the role:

  • Highly developed skills and experience in providing assessment and case management to high need and complex clients
  • Ability to liaise and consult with medical, allied health and other community professionals at all levels
  • Highly developed written and verbal communication skills, with the ability to write reports and prepare correspondence
  • Maintaining knowledge of and working within Organisation Policies and Procedures including WHS, EEO and Code of Conduct
  • The incumbent is responsible for completing duties and responsibilities and ensuring a high level of service efficiency.

Required Certifications & Qualifications:

  • Tertiary qualifications in Allied Health, Nursing or other related discipline coupled with relevant experience
  • Knowledge of Home Care Packages and Aged Care Reform
  • Current driver's licence.
