Working in a supportive team environment, you will be responsible for the delivery of Empower Healthcare's comprehensive services across Home Care and NDIS clients in Perth. You will:
- Conduct comprehensive assessments and deliver tailored physiotherapy treatment plans for clients in their own home through Home Care packages/NDIS.
- Work holistically with other allied health professionals to provide a multidisciplinary care plan
- Educate clients and their families to optimise their ability to live healthy lives within the community
- Liaise with service providers to ensure effective communication
- Work autonomously to prioritize and complete tasks, maintain records and provide patient reports
Do you have what it takes?
- Bachelor of Physiotherapy (or equivalent)
- Registration with AHPRA
- Police Check Clearance
- Excellent verbal and written communication skills
- Strong time-management skills and attention to detail
