What you'll be doing
  • Home Support Assessors provide basic home-based face-to-face assessments of clients referred via the My Aged Care Gateway using the National Screening and Assessment Form (NSAF).
  • Working with each client, the Home Support Assessor establishes goals specific to the client; develops appropriate strategies and actions, including referral to age care and non-aged care services; and develops a Support Plan.
  • The Home Support Assessor completes the NSAF and matches and refers clients to appropriate service(s).
  • You will work collaboratively with clients to access services under the Commonwealth Home Support Program (My Aged Care). This is done through completing a face to face or over the phone assessment, which is used to link clients to supports such as assistive technology, social support services and community care.
This is a part time role, with flexible days, working 8 hour days. You will be supported by the Service Team Leader and other experienced assessors. My Aged Care-specific training is provided on the job.
Where you'll be working
Working for the Central Coast Local Health District, you will be visiting clients across the Central Coast, completing an entry level assessment. You will start based out of Citigate, Wyoming.
