Northern NSW Local Health District is seeking Home Support Assessors to join their Regional Assessment Service. Providing basic home based face to face assessments of clients referred via the My Aged Care Gateway using the National Screening and Assessment Form (NSAF). Working with each client, the Home Support Assessor establishes goals specific to the client; develops appropriate strategies and actions, including referral to age care and non aged care services; and develops a Support Plan. Completing the NSAF and matching and refering clients to appropriate service(s).
If you have/are:
- Certificate III in Home Care or Aged Care or equivalent work experience
- Ability to plan and coordinate client care independently and operate effectively within a multidisciplinary team and identify when client safety is at risk and escalate accordingly
- Strong organisational skills and ability to meet deadlines
- Excellent communication skills, including accurate spelling, good comprehension, good verbal skills and the ability to deal courteously with people at all levels
