Why this role matters

The Salvation Army Australia's Homelessness Stream provides high-quality, person-centred services to people at risk, and or experiencing homelessness. We support adults and families, including accompanying children with accommodation, case management, outreach support, financial assistance, connection and referral to other specialist services.

About the role

We are seeking a values-driven and proactive Homelessness Outreach Case Manager to provide Outreach Case management support to all cohorts experiencing or at risk of homelessness, with the goal of achieving housing stability, improved social inclusion, connection to support systems and the wider community.

Reporting to the Team Leader, this is a Permanent, Full-Time position based in Surry Hills, NSW. Salary and conditions are in accordance with SCHADS Crisis Level 2.

Please note that some travel may be involved to regional NSW and occasionally, interstate.


You will successfully

  • Support the implementation of the Homelessness Stream Model of Care that underpins services across the country and defines expectations and standards for those we serve, our partners and allies, and the Australian community.
  • Provide effective Outreach Case management.
  • Develop case plans alongside the participant and tailor them according to each Participant's goals, background and needs.
  • Put risk assessments and safety strategies in place prior to outreach support/ home visits.
  • Demonstrate commitment to ongoing professional develop
  • Demonstrate evidence of flexible, proactive, client-centred approach including inclusive practice to case work .
  • Follow mandatory reporting processes in a timely manner in line with legislation.
  • Record and input client information, reports and data collection into the relevant databases consistently.
  • Work effectively with internal/external stakeholders and related agencies.
You will have
  • Tertiary or other qualifications within community services, social work, or related discipline (Minimum Diploma Level).
  • Current paid Working with Children Check (NSW).
  • Current NSW Driver Licence.
  • Previous experience in Homelessness (highly regarded).
  • An understanding of issues faced by people experiencing homelessness.
  • Demonstrated experience building and maintaining collaborative relationships with key service providers.
  • Experience of working with people culturally, linguistically and religiously diverse backgrounds, LGBTQIA+.
  • Highly developed interpersonal skills including the ability to consult, advocate and negotiate for a range of clients and stakeholders.
