About the role
The Case Manager is responsible for supporting Young Parents aged 13-24yrs (outreach), and their children to work towards their case plan goals. The role supports Young Parents with complex needs and behaviours to develop independent parenting and living skills through the provision of integrated, individualised case management, behaviour support and direct parenting and emotional support to ensure best possible outcomes for children.
What you will bring
Sound casework/case management experience including holistic assessment of client needs and development of case plans
Understanding of adolescent development and best practice working with young women and men with complex needs and behaviours and demonstrated sensitivity to the needs and issues facing Young Parents and their children
Experience in working with people in the community who have complex needs and experiences
Experience in working competently with staff and clients from cultural and linguistically diverse backgrounds with respect and humility
Experience in working respectfully and competently with Aboriginal and Torres Strait Islander people and communities
Working with children’s check
Drivers licence
Residential Care Workers Register registration

This role is covered by the Social, Community, Home Care and Disability Services Award - Level 4. Salary from $82,063.28 FTE + superannuation + access to salary packaging.
