Your Opportunity
The South Western Sydney Early Intervention and Tenancy Support Service requires an Outreach Case Manager to join the team working with approximately 25 to 30 clients across Bankstown, Liverpool, Fairfield, Campbelltown, Wingecarribee and Wollondilly. The role supports to adults and families providing case management and support with housing with a goal to prevent homelessness.
Your key responsibilities will be to:
- Work with and respond to referrals of clients across Liverpool and Fairfield providing ongoing case management and support
- Create individualized support plans including referrals to community services organisations
- Promotion of the service in the local community
- Monitor our clients progress towards achieving their goals
To be successful in this role, you will need to possess:
- Ideally Diploma in Social Welfare or relevant industry experience
- Senior First Aid Certificate
- Current drivers licence. Comprehensively insured vehicle for possible business use.
