About the opportunity
We have an exciting opportunity for two Lead Family Practitioners to join our Homes 4 Families (H4F) Program. The H4F Program delivers housing and family services support to children and families who were provided with emergency hotel accommodation after experiencing homelessness during the COVID-19 pandemic. The program provides intensive and flexible wraparound case management support to assist these families to transition into stable, safe longer-term housing.
As a Lead Family Practitioner, you will provide long-term case management support to a caseload of approximately 6-8 families as they move out of hotels and into longer-term homes. The Practitioner will utilise a strengths based, child and family centred approach to enhance the safety, wellbeing and stability of the children and family unit. The role will undertake risk assessments, provide assertive outreach, develop Family Action Plans, facilitate care team planning and support families to engage with local community networks.

About you
To be successful in your application you will meet the person specification in the position description, including having a degree in social work, psychology or a child and family welfare related discipline.
You will also have:
- Demonstrated case management experience within the child, youth and family welfare sector
- Demonstrated understanding of the Best Interest Principles and the Children, Youth and Families Act 2005
- An understanding of the challenges and stigma faced by people experiencing homelessness
- A strengths-based, family-centered approach to case management
- Sound risk assessment and risk management skills
- Strong communication skills, both written and verbal
