Where you'll be working
The position is based at Royal North Shore Community Health Centre in St Leonards and is, at times, required to travel across Northern Sydney, Northern Beaches and Hornsby Ku-ring-gai local government areas.
What you'll be doing
The Youth Health and Wellbeing Pathways (YHWP) Service Manager provides leadership and overall operational management for the following services:
- Out of home care - Health Pathway Program
- Vulnerable adolescents and young adults (12 to 24 years) and their families
- Aboriginal and Torres Strait Islander Targeted Early Intervention (TEI) Program for young people.
Aboriginal and Torres Strait Islander People are encouraged to apply.
- Describe your experience working with adolescents and young people in a community context and how you would apply your knowledge in this role.
- Please describe at least one example of a successful interagency partnership where you have played a key role in developing health services for young people.
- Describe a quality improvement, service planning or health promotion initiative where you have taken a lead role. Please include a brief overview of the initiative's evaluation and outcomes.
