Form of workMiChoice Disability Supports is a registered NDIS provider, supporting people with Mental Health and disability in the Toowoomba, Warwick, Western Downs, Brisbane, and Ipswich areas. We implement individualised supports and services aimed at developing and maximising autonomous living opportunities, increased social and recreational skills, including community and social inclusion. Additionally, independence, supported decision making and individual personal choice is promoted through an educational life skill path.
Responsibilities and Duties
- Provide direct support and care to clients in accordance with their individual Support Care Plan as well as their NDIS Funding Support Plan
- Monitor the safety and well-being of the clients and report any concerns through appropriate channels.
- Liaise comprehensively with your colleagues ensuring consistency is across all systems of operation through an open communication pathway.
- Adhere to all MiChoice Disability Supports, Management Systems of policies and procedures
- Understand individuals’ disability as their needs change through communication, observation, and Support Care Plan Management
