About the program...
Your LINC (Lived Identity Navigate Connect) is a multi-agency, two-tier Mental Health outreach service delivered in partnerships with Centacare and KWY Aboriginal Corporation. Our service offers holistic, flexible and culturally responsive phyco-social support for individuals experiencing Mental Health challenges and distress across Metropolitan Adelaide and some peri urban post codes.
KWY provides non-acute psychosocial support through the provision of case management to support people to build independence connect with community and improve quality of life over a period of 4 – 12 months.
About the role...
The role involves providing the direct support and case management to Aboriginal and Torres Strait Islander people who provided non-acute psychosocial services and intensive crisis support services. Mental Health Practitioners are responsible for guiding the delivery of the Your LINC service for KWY across Metropolitan Adelaide and will report directly to Program Coordinator.
About you...
You have worked in psychosocial and social and emotional wellbeing for Aboriginal and Torres Strait Islander people and are able to work with various stakeholders internal and external, within a multi-agency team. You have an understanding of the barriers experienced by Aboriginal and Torres Strait Islander people in accessing mainstream Mental Health services and are familiar with working within a Social and Emotional Wellbeing Framework.
About the pay...
- Positions are classified under the SCHADS award
- Salary range is $82,045 - $88,277 Full Time Equivalent excluding superannuation

Other requirements…
- You must hold a current South Australian Drivers License or a license which permits you to drive in South Australia.
- Successful candidates will be required to clear and maintain probity checks including National Criminal History Record Check (Police Clearance), Working with Vulnerable People Check and Working with Children Check (applicants need to hold a current WWCC to apply).
