- Unique opportunity as a NDIS Support Coordinator Team Leader in our supportive, fun, friendly, and collaborative NDIS and Disability Program

- Classification: Social Worker Grade 3, Year 4 (SC34)*
- Provision of leadership and strategic direction in collaboration with NDIS and Disability Program Manager in relation to delivery of Support Coordination to people with disabilities in our local area
- Responsibility over financial viability in collaboration with the NDIS and Disability Program Manager, including staff and resource management, monitoring of KPI's, participant service agreements and operational efficiency and effectiveness
- Ensure Peninsula Health services are in line with the NDIS Quality and Safety Commission requirements for provider registration, including preparation and completion of external audit against the NDIS Quality and Practice Standards.
- Partnering with external agencies including the Brotherhood of St Laurence, NDIA and other NDIS and health service providers to ensure positive experiences and outcomes for our participants
- Service evaluation, including consumer experience and outcomes, and consultation into the development of a 3 year strategic business plan to meet the evolving consumer and service needs
We anticipate that you will have:
- Demonstrated current specialist knowledge and skills in the provision of Support Coordination
- An understanding of legislation and standards relevant to the disability sector and underpinning the NDIS?
- Strong proactive and collaborative leadership
- Demonstrated clinical leadership and supervision of staff to promote professional competence and foster individual staff development
- Highly developed communication skills including negotiation, conflict resolution, supporting change processes, oral presentations and creative problem solving
- A current and valid Victorian Driver's Licence
- A current or if a preferred candidate willing to apply for a NDIS Workers Screening Check at your own expense
