Position Overview
We are currently seeking a passionate and proactive Casual NDIS Support Coordinator to join our team in Top Ryde, Sydney. As a Support Coordinator, you will play a vital role in assisting NDIS participants to navigate the scheme, access appropriate services, and work towards their goals outlined in their NDIS plans.
Key Responsibilities:
- Work closely with NDIS participants to understand their goals, needs, and aspirations.
- Assist participants in developing and implementing their NDIS plans, ensuring they have choice and control over their supports.
- Identify and connect participants with suitable service providers based on their individual needs and preferences.
- Liaise with families, carers, and support networks to ensure coordinated and holistic support for participants.
- Advocate for participants' rights and ensure their voice is heard in decision-making processes.
- Monitor participants' progress towards their goals and make necessary adjustments to their plans.
- Maintain accurate and up-to-date records, reports, and documentation in accordance with NDIS guidelines.
- Collaborate with allied health professionals, community organizations, and government agencies to facilitate comprehensive support for participants.
- Provide information and education to participants and their support networks about the NDIS and available services.
Qualifications and Skills:
- Relevant qualification in disability services, social work, community services, allied health, or a related field.
- Demonstrated understanding of the NDIS, including legislation, policies, and processes.
- Experience working in the disability sector and supporting individuals with diverse needs.
- Strong interpersonal and communication skills with the ability to build rapport and maintain effective relationships.
- Excellent organizational skills and the ability to manage multiple tasks and priorities.
- Problem-solving abilities and a proactive approach to addressing challenges.
- Empathy, patience, and a genuine commitment to advocating for the rights and needs of individuals with disabilities.
- Computer literacy and proficiency in using relevant software for documentation and reporting.
- Valid driver's license and access to a vehicle (preferred).
