Key responsibilities
- Provide Level 2 Coordination of Supports to NDIS participants.
- Complete plan review reports, support plans, case notes, and other relevant required reports.
- Monitor and report progress against NDIS goal plans and budgets.
- Knowledge and understanding of the NDIS (e.g., policies & price guide) and the Quality and Safeguarding Framework.
- Collaborate with participants and their informal support networks and build connections with providers, local community services and NDIS contacts.
- Support participants in understanding their NDIS plan and funding plan.
- Support participants in designing their support framework within available plan funds to achieve goals, including coordinating the participant's budget.
- Gather and prepare evidence for the participant's plan review.
- Perform all underlying administrative tasks related to support coordination, service provision and budget.
- Keep up with tasks within the timeframe, manage caseload and deadlines, and work within the Key Performance Indicator framework.
- Develop strong work relations with relevant internal and external stakeholders, including communities and government entities such as the Department of Justice, health, education, employment, community NFPs, and other NDIS providers.
- Engage with NDIA regional contacts, including Local Area Coordinators.
- Maintain the privacy and confidentiality of participants, carers, families, and the organisation.
- Demonstrated minimum 2 years experience as a NDIS Support Coordinator or relevant experience in a similar field.
- Provide proofing evidence of Knowledge (NDIS plan, budgeting, and invoicing), such as a letter of appreciation from a previous workplace.
- Diploma level is desirable.
- Experience with Mental Health and Disability is essential
- Experience coordinating a broad range of community service programs, including the NDIS
- Show the ability to coordinate service expectations and professional boundaries.
- Strong communication skills, including listening.
- Able to research, interpret and apply complex concepts
- Able to apply complex concepts to design and implement practical solutions for participants within plan funding.
- You can coordinate, organise, and track activities/bookings/tasks
- Able to self-manage to achieve multiple and often competing deadlines
