About the program

Carer Gateway was first introduced in 2015 as a website and phone line to help carers find and access support to help them in their caring role. With the Australian Government’s commitment to continue improving carer services, an extensive four-year consultation process with carers and the sector was undertaken.

This process found that the best way to support carers is to help them early in their caring role. Through a range of tailored supports and services, carers can be supported to manage their daily challenges, reduce stress, plan for the future and ultimately improve their wellbeing.

About the opportunity

The Non-clinical Palliative Carer Support Planner is responsible for undertaking intake, assessment and planning services for carers accessing support provided by the Carer Gateway across South West Sydney, Nepean Blue Mountains and Queensland.

This role will have a specific focus on Carers and/or care recipients who have a palliative diagnosis, to assist in accessing appropriate services. The objective of the role is work with carers over the phone to access information, support and referrals relevant to their caring role. This role may also include an outreach and face to face component.

Reporting to the Coordinator working under limited direction, this role will involve:

  • Supporting Carers and/or Care Recipients with a palliative diagnosis with a high level of customer service, ensuring carers are listened to with patience and non-judgement.
  • Provide Intake, Planning and Assessment services to Carers as outlined in the Carer Support Framework.
  • Assisting other areas of the business when required to manage call overflows with inbound calls throughout the contact centre.
  • Working with Carers to establish and implement agreed action plans.
  • Assist carers in navigating and accessing palliative services.
  • Support carers and their care recipients to link to other Government funding streams, including My Aged Care, End of Life packages, Adolescent and Young Adult Hospice, NSW Paediatric Palliative Care Program, Palliative Care QLD, and more.
  • Ensuring Carers have an understanding of the palliative diagnosis and linking them onto educational services if not to include codesigning and providing specialist resources and education.
  • Developing partnerships for warm referrals to specialist services.
  • Supporting Carers and/or Care Recipients to have completed an Advance Health Directive.
  • Working in collaboration with Wellways Carer Gateway Community Engagement Officers and Aged Care Navigators.
  • Provide face to face, and outreach supports where identified.
  • Ensuring the service approach incorporates the following service principles: Carer Focussed, Practicality and Flexibility, Inclusiveness.
  • Creating an environment which Carers will find welcoming, real and inclusive.
  • Develop support relationships with Carers which are positive and are built on Wellways values.
  • The Palliative Care Specialist role is not limited to the listed responsibilities and will require assistance with other areas of the program to meet the requirements of the organisation.

To be successful in this role, you will need:

  • Qualifications and/or experience working in community services, allied health or carer/family peer positions.
  • Certificate of Palliative Care, Grief and bereavement and/or other palliative care training courses.
  • Excellent communication and interpersonal skills
  • Right to work within Australia
  • Current valid Driver’s Licence and the ability to undertake some travel
  • NDIS Worker Orientation Module Certificate
  • Satisfactory National Police Records Check, NDIS Worker Screening Check and Working with Children's Check
