About the role
Employees classified at this level provide clinical nursing/midwifery expertise for specified individual patients/clients and/or groups/populations, and/or provide clinical leadership for an area of practice and may work in a variety of clinical settings. Employees classified at this level are experts within their area and stream of practice and have a significant degree of autonomy and decision making.
This position is required to possess and maintain contemporary knowledge about hospital avoidance and supported discharge strategies, including both internal and external services, to support and coordinate safe patient discharges, including referrals to home and community-based services, to assist in preventing unnecessary representations and admissions to CALHN hospitals.
This position will act as a CALHN Hospital Avoidance and Supported Discharge (HASDS) expert, by providing clinical leadership and education within the organisation, as well as establishing and maintaining effective working relationships within CALHN and relevant Government and Non-Government Organisations (NGO), to identify any enablers or barriers for hospital avoidance and supported discharge service utilisation.
This position will provide support to the Team Manager to assist with leadership of the Multidisciplinary team, daily operational activities (including capacity management), development and implementation of pathways, supporting strategic objectives, and supporting safety and quality initiatives.
The position will be required to work across different sites within CALHN integrated Care and a 7 day roster.
Qualifications
- Registered or eligible for registration as a Nurse by the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate.
