About the Role
This is an exciting opportunity for a Nurse Practitioner to join our Street Doctor street-side mobile medical service. Black Swan Health’s Street Doctor aims to improve health outcomes for people experiencing disadvantage and marginalisation. Our RACGP accredited Street Doctor service is provided by General Practitioners, supported by Nurse Practitioners, Registered Nurses and Outreach Workers, and offering people the highest quality health care.
Black Swan Health’s Street Doctor clinics are delivered in diverse and interesting locations across the north and south metropolitan areas including Fremantle, Scarborough and other north of the river locations.
The Nurse Practitioner will provide comprehensive nursing care, within scope of practice for Nurse Practitioners to Freo Street Doctor Clients.
The successful candidate will:
- Provide comprehensive nursing care, within scope of practice for Nurse Practitioners.
- Ensure continuity of client care through collaboration with other members of the Freo Street Doctor team and other health professionals or services.
- In collaboration with supporting GP develop and receive approval for Health Care Plans, including mental health care plans, to expedite patient access to supporting health services.
- Maintain an understanding and operation of the medical software programs used by the service.
- Hold a current general registration AHPRA.
- Hold a current a general registration with the Nurses and Midwifery Board of Australia as a registered nurse with endorsement as a Nurse Practitioner.
- Eligible to apply for Medicare Provider Number
- Current CPR Certification
- Current National Police Clearance
- Current Working with Childrens Check
- Current eligibility to work in Australia, with no restrictions.
